NDIS Social Work Services | COSY Coordination
COSY Coordination provides NDIS-funded Social Work services in line with the NDIS Practice Standards and Capacity Building Supports – Improved Daily Living.
Our Social Workers deliver evidence-informed, goal-focused supports that assist participants to build independence, strengthen wellbeing, and develop the skills needed to participate more fully in daily life and the community.
All services are delivered in accordance with:
- NDIS Practice Standards
- NDIS Code of Conduct
- Participant goals outlined in the NDIS plan
What is NDIS Social Work?
NDIS Social Work is a Capacity Building support designed to help participants:
- Improve functional capacity
- Strengthen social participation
- Build skills that support emotional wellbeing
- Support informed decision-making
- Manage complex personal and social circumstances
NDIS Social Work focuses on outcomes that build independence and can reduce reliance on long-term supports over time.
NDIS-aligned Social Work supports at COSY Coordination
Our Social Workers provide supports within the NDIS framework, including:
Psychosocial and functional assessment
Assessment may consider a participant’s:
- Social and emotional wellbeing
- Living situation and current supports
- Risk and safeguarding needs
- Barriers to participation
- Capacity building goals
Assessments help inform structured supports linked to NDIS goals.
Capacity Building – Improved Daily Living supports
Supports may include assistance to:
- Develop coping strategies and emotional regulation skills
- Improve communication and interpersonal skills
- Build routines and daily living strategies
- Increase confidence and self-advocacy
- Strengthen problem-solving and decision-making
- Improve participation in social and community activities
All supports remain goal-oriented and outcomes focused.
Support with complex needs and life transitions
Social Workers may support participants experiencing:
- Psychosocial disability
- Housing instability or transitions
- Family breakdown or conflict
- Grief and loss
- Hospital discharge or care transitions
- Periods of increased vulnerability
Supports are provided within the scope of NDIS funding and aligned to participant goals.
Support with complex needs and life transitions
Social Workers can support participants to:
- Understand their NDIS plan and funded supports
- Prepare for plan reviews (capacity-building support)
- Engage effectively with service providers
- Access appropriate mainstream and community services
- Build skills to self-advocate over time
This support focuses on building the participant’s capacity to navigate systems more independently.
Advocacy and system navigation
In line with the NDIS Practice Standards, COSY Coordination Social Workers can provide:
- Risk and safety considerations (within role scope)
- Support planning to reduce vulnerability
- Referral pathways to appropriate services
- Duty of care and safeguarding responses
- Trauma-informed and ethical practice
Family and carer support
Where linked to a participant’s goals, Social Work supports may include:
- Education and guidance for families and carers
- Support with boundaries and communication
- Strengthening informal support networks
- Reducing carer stress and burnout
Who can access NDIS Social Work services?
NDIS Social Work services may suit:
- Participants with psychosocial disability
- Participants with complex support needs
- Participants experiencing significant life transitions
- Participants requiring capacity building supports
- Families and carers supporting an NDIS participant
COSY delivers services using a person-centred, culturally safe and trauma-informed approach.
How services are delivered
COSY Coordination Social Work services may be delivered:
- In the participant’s home
- In the community
- Via telehealth (phone or video)
- At an agreed safe location
Service delivery is flexible and guided by participant choice and control.
